COVID-19 News: Since the onset of the COVID-19 pandemic in late 2019, an extensive global vaccination campaign has swiftly ensued. As with any vaccine, the COVID-19 vaccine carries potential side effects. Common adverse reactions include temporary headache, fever, and muscle pain. Additionally, the vaccine can impact endocrine organs. Notably, thyroid and pancreas disorders have been reported as major SARS-CoV-2 vaccine-related endocrinopathies since the inception of the vaccination effort.

Pituitary ailments induced by SARS-CoV-2 vaccination have increasingly surfaced in medical literature and even in
COVID-19 News coverages. A thorough search on PubMed/MEDLINE for commentaries, case reports, and case series detailing pituitary disorders following SARS-CoV-2 vaccination yielded a number of cases reports.
None of the cases had a personal or familial history of pituitary disease, and all patients had no prior SARS-CoV-2 infection. Most of the patients received the mRNA BNT162b2 (Pfizer–BioNTech) vaccine.

The spectrum of pituitary disorders included hypophysitis (with varying clinical presentations from pituitary lesions to thickened pituitary stalks and pituitary apoplexy. The onset of pituitary disorder ranged from one to seven days post-vaccination.
Unlike thyroid disorders, pituitary disorders manifest rapidly after COVID-19 vaccination (within seven days compared to two months for thyroid diseases), likely due to the pituitary gland's small size and rapid dissemination of vaccine components. These preliminary findings underscore the importance for clinicians to remain vigilant for potential pituitary adverse effects of vaccination. This awareness will aid in promptly directing patients for medical evaluation when presenting with concerning symptoms such as fatigue, excessive thirst and urination, or severe headaches following COVID-19 vaccination.
While some claim that these mRNA COVID-19 vaccine induced pituitary disorders are rare, it should be noted that most of the case studies emerged as a result of individuals exhibiting severe symptoms in the late stage of their condition. To date, there has been no study covering a large cohort to analyze the state of various individual’s pituitary gland after the mRNA vaccines. We believe that if such as study was conducted, it would reveal shocking findings!
COVID-19 mRNA Vaccine Induced Hypophysitis
Hypophysitis is defined as inflammation of the pituitary gland that is primary or secondary to a local or systemic process. Differential diagnosis is broad (including primary tumors, metastases, and lympho-proliferative diseases) and multifaceted.
Symptoms related to all types of hypophysitis include frequent headaches, vision changes (either low vision or double vision), and impaired pituitary function. Many other symptoms may materialize as a result of the hormones affected.
In hypophysitis, a condition affecting the pituitary gland, hormone levels from the anterior portion like ACTH, TSH, growth hormone, and sexual hormones typically decrease, while prolactin levels may fluctuate. Involvement of the posterior gland or pituitary stem can lead to diabetes insipidus, characterized by excessive thirst and urination due to a lack of antidiuretic hormone.
The hormone deficiencies manifest in various symptoms: decreased stress response with low ACTH, fatigue and cold intolerance with low TSH, sexual dysfunction and infertility with decreased sexual hormones. Severe ACTH deficiency can be life-threatening. Low prolactin reduces milk production, while high levels can disrupt menstruation and fertility. Diabetes insipidus results in excessive thirst and urination due to inadequate antidiuretic hormone.
Furthermore, hypophysitis may halt the production of ACTH, essential for cortisol release, impacting stress response, appetite, sleep, and organ functions. Severe deficiency in ACTH and cortisol can be fatal. Secondary symptoms include increased urination and thirst, sexual dysfunction, weight loss, fatigue, and high blood sodium levels. Additional symptoms may include nausea, vomiting, and excessive prolactin production.
Often hypophysitis is not easy to diagnose. A diagnosis of hypophysitis is made by completing laboratory tests to confirm the abnormal hormone levels in the blood and by imaging studies, usually magnetic resonance imagining (MRI) with contrast. MRI abnormalities may include diffuse enlargement and/or homogeneous contrast enhancement of the pituitary gland, as well as thickening of the pituitary stalk.
In cases of secondary hypophysitis, additional tests will be needed depending on the suspected etiology.

COVID-19 mRNA Vaccine Induced Pituitary Apoplexy
Pituitary apoplexy is an infarction or hemorrhage of the pituitary gland. Pituitary apoplexy occurs when there’s a blockage in blood flow or bleeding (hemorrhage) in your pituitary gland. It can be a serious condition, requiring emergency treatment. This condition sometimes involves a pre-existing pituitary adenoma.
Typically, symptoms of pituitary apoplexy depend on the extent of the:
-Bleeding.
-Cell death of pituitary gland tissue from the lack of blood flow.
-Swelling (edema).
The most common sign is a sudden and severe headache. More than half of people also have vision problems due to increased pressure on the nerves that run from your brain to your eyes. These may include:
-Drooping eyelid.
-Double vision.
-Partial or full vision loss in one or both eyes.
However, some other types of symptoms are caused by changes in the brain. These can resemble other neurological conditions, such as meningitis. These symptoms include:
-Confusion.
-Fatigue.
-Fever.
-Nausea and vomiting.
-Sensitivity to light (photophobia).
Pituitary apoplexy can also affect hormonal function. Damage to the pituitary gland affects the function of other endocrine glands, the hormones they produce and the bodily processes they control.
A common hormonal complication is adrenocorticotropic hormone (ACTH) deficiency. This can affect the function of the adrenal glands and can be life-threatening. Its symptoms include:
-Low blood sugar (hypoglycemia).
-Low blood pressure (hypotension).
Pituitary apoplexy is typically diagnosed by magnetic resonance imaging (MRI).

COVID-19 Vaccine-Induced Expansion of Pituitary Adenoma
Pituitary adenomas are benign tumors of the pituitary gland. Most are located in the anterior lobe (front portion) of the gland. About 1 in 10 people will develop a pituitary adenoma in their lifetime. Some pituitary adenomas secrete one or more hormones in excess.
A past case report had shown that the COVID-19 mRNA vaccines can cause enlargements of these pituitary adenomas.
