Thailand Medical: While Thailand has so far found only one case of the Mpox Clade 1b involving a European traveler who was actually on transit flight, it is actually facing a growing challenge in controlling the local transmission of Mpox Clade 2b, with 142 cases recorded so far in 2024. The total number of infections since the virus made its global debut in 2022 has reached 833. The situation has become particularly concerning as 30 new cases were reported in July and August 2024 alone.

Public health experts are now calling for stronger measures from
Thailand Medical authorities to prevent further spread.
Incomplete Data May Obscure the Full Picture
The official numbers could be underreported, as many individuals seek treatment at private hospitals and clinics that may not report data to government authorities. Some people are opting for anonymous clinics, while others, in a bid to avoid stigma, resort to self-treatment without proper medical supervision. This self-treatment often involves using home remedies to manage visible symptoms, such as skin lesions, without addressing the underlying infection. These individuals remain contagious and may unknowingly contribute to the spread of the virus.
Low Vaccine Uptake: A Financial Barrier
One of the main challenges in controlling Mpox in Thailand is the low uptake of vaccines. The cost of vaccination has become a significant hurdle for many people. The Thai Red Cross is currently offering a single dose for 8,500 Baht (approximately USD 230), not including additional fees.
A full vaccination requires two doses, which makes the total cost prohibitive for many, especially for those in at-risk communities. Private hospitals charge even higher fees, further limiting access to the vaccine.
With limited vaccine coverage, the population remains vulnerable, particularly in high-risk groups. Authorities are urged to either subsidize the cost of the vaccine or offer it for free, especially to vulnerable populations such as sex workers and those engaging in high-risk behaviors.
A Growing Trend in Infections Among Young Males
Mpox Clade 2b infections in Thailand are predominantly affecting young males, particularly those between the ages of 20 and 40. A large proportion of these individuals are gay men, with a significant number working as sex workers. This demographic has been at the epicenter of the outbreak, raising concerns about the role of unprotected sexual contact in the transmission of the virus.
However, there is a worrying new trend. Women, particularly those involved in the sex trade or in relationships with bisexual men, are now contracting the virus. This shift underscores the need for broader public health interventions that go beyond the gay community and target all individuals at risk of exposure.

Hotspots for Infections: Bangkok and Surrounding Areas
The majority of Mpox infections are concentrated in Bangkok, with significant clusters also emerging in the provinces of Chonburi, Nonthaburi, and Samut Prakarn. These areas are home to many high-risk venues, such as gay saunas, spas, and bars, where the virus may be spreading. Without adequate preventive measures, including routine screenings and public education campaigns, the virus could continue to circulate in these communities unchecked.
Public Health Authorities Urged to Take Stronger Action
Public health experts are calling on the Thai government to ramp up efforts to control the spread of Mpox Clade 2b. A more robust response is required, particularly in terms of public education and accessible healthcare services for at-risk populations. Authorities are encouraged to conduct more outreach campaigns, particularly in gay establishments such as saunas, bars, and spas. Offering free health screenings and encouraging safe sexual practices through condom distribution and education could also help curb the spread of the virus.
Another essential measure would be to provide free or subsidized Mpox vaccines, especially to sex workers and others at high risk. Without these interventions, the number of cases could continue to rise, leading to a more severe public health crisis.
A Call for Greater Awareness and Support
As Thailand grapples with the growing Mpox outbreak, the onus is on both the government and the public to take preventive actions. The general population, particularly those in high-risk groups, must be educated about the risks of transmission, the importance of early detection, and the need for proper medical treatment. Only through coordinated efforts between authorities, healthcare providers, and the community can Thailand effectively curb the spread of Mpox Clade 2b and protect its citizens from further infections.
